This Coaching Contract (“Contract”) is entered into by and between Titan Shape, 303 Main St, Little Elm, TX 75068 (“Trainer”), and the undersigned (“Client”).

2. Terms & Conditions

• Schedule: Trainer and Client shall agree on regular online check-ins.

• Payment: Client shall pay the agreed monthly fee; all payments are non-refundable.

• Cancellation: Client must provide 30 days’ written notice to cancel services. Any prepaid fees beyond the cancellation date will be prorated and refunded within 7–10 days. Failure to provide notice forfeits remaining fees.

• Meeting Changes: 24 hours’ notice is required to reschedule or cancel a check-in; otherwise, the session may be forfeited.

• Health: The undersigned affirms they are medically fit to participate and shall immediately inform Trainer of any health changes.

• Liability Waiver: The undersigned acknowledges the inherent risks of physical activity and releases the Trainer from any liability for injuries incurred during or as a result of the program.

• Confidentiality: All Client information shall remain confidential except as required by law.

• Termination: Either party may terminate this Contract with 30 days’ written notice.
